APPOINTMENTS
Patients are seen by appointment. This enables us to work as efficiently as possible. We value your time and want to avoid long waiting periods. Please understand that medical emergencies may occasionally cause delays and in these instances we ask for your understanding.
If you cannot keep an appointment we kindly ask that at least 24 hours notice be given to the office. This makes it possible to give that time to another patient.
Please arrive 15 minutes before your first scheduled appointment so you have time to complete the patient information sheet, history and physical forms.
Those patients with Internet access can download these forms and fill them out ahead of time. It is important that the information you provide be accurate, since we will rely on this data as you make future visits. If there is a change of address, telephone number, etc please inform us at your next visit. We also will need a copy of your insurance card.

All office visits are to be paid at the time of service as well as any copay that is required by your insurance company. For your convenience, we accept Visa and Mastercard.
EMERGENCIES
If you have a life-threatening emergency, call 911 IMMEDIATELY. Otherwise, our physicians can be reached by calling the office telephone number (970) 669-3212 day or night. Rarely, the on-call physician may be unable to respond immediately because he is in surgery. If you are unable to reach the on-call physician in a true emergency, please go directly to McKee Medical Center's Emergency Department.
OUR STAFF
Our staff are well-trained professionals who promote a courteous and caring environment. The medical staff as well as the patient accounting department are available during all regular business hours.
Any questions or concerns regarding Loveland Surgical Associates can be directed to Terri Davis, Practice Manager/Privacy and Security Officer, who will address them as quickly as possible.
