Appointments

Patients are seen by appointment. This enables us to work as efficiently as possible. We value your time and want you to avoid long waiting periods. Please understand that surgical emergencies may occasionally cause delays, and in these instances we ask for your understanding. If your surgeon has an unexpected emergency, we attempt to contact you to let you know of possible delays and to arrange an alternate time or date to see your surgeon. Please be sure our office staff has an up to date contact number to reach you should this occur.

If you cannot keep an appointment, we kindly ask that you give our office a 24-hour notice. This makes it possible to give that time to another patient.

Please arrive 15 minutes before your first scheduled appointment so you have time to complete the patient information sheet, history and physical forms. For most follow up visits, you can arrive at the scheduled time. Our goal is to see you on time. If you arrive at the office and have had to wait more than 10 minutes past your scheduled time, please clarify the delay with our receptionist.

Those patients with Internet access can download the appropriate check in forms and fill them out ahead of time. It is important that the information you provide be accurate, since we will rely on this data to make appropriate surgical recommendations. If there is a change of address, telephone number, new medical diagnosis, new medications, etc., please inform us at your next visit. We also will need a copy of your insurance card on most visits.

To make an appointment, call the office at (970) 669-3212 during normal office hours, Monday-Thursday 8:30 am to 5:00 pm, Friday 8:30 am to 4:00 pm

All office visits are to be paid for at the time of service, as well as any copay that is required by your insurance company. For your convenience, we accept Visa and MasterCard.

Emergencies

If you have a life-threatening emergency, call 911 IMMEDIATELY. Otherwise, our physicians can be reached by calling the office telephone number, (970) 669-3212 day or night. Rarely, the on-call physician may be unable to respond immediately because he is in surgery. If you are unable to reach the on-call physician in a true emergency, please go directly to your hospital’s Emergency Department.
